Policy

All new patients must have a referral from a physician or complete our new patient paperwork for Dr. Brown's review. Please bring your ID and insurance card to all visits. Co-pays are due at the time of service. Please notify our office if you need to cancel or reschedule; if not done at least 24 hours prior to your appointment, a no-show fee will be assessed.
